Notice of Rights and Authorization to Release Confidential Education Records
As a student of Lakeshore Technical College, I understand that I have the right, upon written request to Student Services, to inspect and/or review the materials in my student record. The exceptions to my right of review, under the regulations implementing the federal Family Educational Rights and Privacy Act (FERPA), 20 U.S.C. 1232g, et. seq., include:
- Records that contain information on a student other than myself
- Financial records, including any information those records contain, of my parent(s)
- Confidential letters/statements of recommendation placed in my file before January 1, 1975, as long as those letters/statements are used only for the purposes for which they are specifically intended
- Confidential letters/statements of recommendation placed in my file after January 1, 1975, if:
- I waived my right to inspect or review these letters/statements
- The letters/statements are related to my:
- Admission to an educational institution;
- Application for employment; or
- Receipt of an honor or honorary recognition
- Any document which is not an education record within the definition of Title 34 of the Code of Federal Regulations Section 99.3, including records of instructional, supervisory, and administrative personnel which are in their sole possession and for their exclusive use.
I understand that Lakeshore Technical College will provide access to my educational records within 45 days of my written request.
I hereby authorize the Wisconsin Hospital Preparedness Program to receive a copy of the following education records:
Registration Status and Progress reports
Wisconsin Hospital Preparedness Program
Wisconsin Division of Public Health
